Is there any website where I can create a survey for my friends to fill out and post it on myspace?

I am looking for a website to create my own survey that my friends can fill out and the results can be sent to my email address or something. I tried a lot of them from google but it just didn't seem to work out right. I don't want the whole quiz to be on myspace, Just a link or something. This would be really helpful. Thanks.
